Shropshire Community Health NHS Trust is a new NHS Trust for Shropshire which brings together the community health services of NHS Telford and Wrekin and Shropshire County Primary Care Trust into a single organisation.
As we are bringing together two organisations into one, sometimes the services may be slightly different across the two former PCT areas of Shropshire County and Telford and Wrekin, and you may see different contact details. We are working to bring services together to share the best of both, while still recognising the particular needs of different areas.
