Preventing Falls
The Falls Prevention Service aims to reduce the number of falls that patients suffer. We also aim to reduce the number of possible fractures caused by falls.
The service is for people who are registered with a GP in Shropshire and Telford and Wrekin. We assess people who have fallen, are at risk of falling, or are fearful of falling, and we offer a range of ways to lower an individual’s risk of falls and to help people stay healthy, active and independent. We may also signpost people to other teams and services within the community.
We also raise awareness and provide training about falls to patients, health and social care professionals, private and voluntary agencies. This includes how to prevent falls and how to cope with daily life after a fall.
Access to the service is via the Care Coordination Centre (CCC) Falls Prevention Line. You can refer yourself to the service or be referred by a health or social care professional.
